29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  229 N Main Street, N Brookfield, MA: On or about June 30, 2016 an employee was required to use a half face elastomeric respirator when performing spray finishing operations and the employer had not done an evaluation to determine a reasonable estimate of employee exposures prior to requiring the use of respirators.

29 CFR 1910.307(c)(3): Equipment was not safe for the location in that it was not of a type and design that the employer demonstrated would provide protection from the hazards arising from the combustibility and flammability of vapors, liquids, gases, dusts, or fibers involved:  229 N Main Street, N Brookfield, MA: On or about June 30, 2016 employees were conducting spray finishing operations in an area which had the following electrical equipment that was not of a type and design that would provide protection from hazards arising from combustibility and flammability of vapors, gases, dusts or fibers involved:  (a) Incandescent work light on a pendant. (b) Toggle switch and associated wiring in spray area. (c) Duplex outlet and associated wiring in spray area. (d) Ceiling mount ventilation fan with 13 inch inlet in spray area.
