On or about June 30, 2016, employees were performing roofing activities on a low-slope roof at a height greater than 6 feet with no fall protection. The Alfredo Martenez dba Alfredo Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1111367, citation number 1, item number 2 and was affirmed as a final order on January 13, 2016, with respect to a workplace located at 1105 N. Bequette Street, Dodgeville, WI 53533.
