Location: 24 Lime Street, Newburyport, MA. On or about July 1, 2016, an employee was not protected from falling while working on a roof at a height greater than 23 feet above ground level. The 2 Penn, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1127534, citation number, item number 1 and was affirmed as a final order on March 25, 2015, with respect to a workplace located at Northern Blvd, Newburyport, MA.

On or about July 1, 2016, a non self supporting ladder used to access the roof at a height greater than 23 feet above ground level was not set up at the proper angle.
