Located on the west side of a residential roof structure, there was no frequent and regular inspections being performed by a competent person to ensure materials equipment such as residential fall protection systems and/or devices were being maintained and utilized in a safe condition while employees were engaged in framing work hand signaling. Potential fall height was measured to be greater than 25' above the ground below.
