29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Front of Residence: On or about June 30, 2016 and times prior, two employees were working approximately 15 feet above ground level from the platform of a Genie S-60X aerial lift. The employees were not wearing full body harnesses with lanyards attached to the basket while working from the lift.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b): a) 303 Pennsylvania Ave: On or about June 29, 2016 and times prior, two employees were working on the back of the roof on a two story residence 25 feet eave to ground without the use of fall protection. b) 303 Pennsylvania Ave: On or about June 30, 2016 and times prior, one employee was working on the front of a roof on a two story residence 15 feet eave to ground without the use of fall protection.
