29 CFR 1910.106(e)(5)(v): All plant fire protection facilities were not adequately maintained and periodically inspected and tested to make sure they were always in satisfactory operating condition, and they could serve their purpose in time of emergency. a. On or about June 29, 2016, at the isopropanol loading station, an Aqueous Film Forming Foam supply hose to the fixed fire suppression cannon was kinked and crushed, exposing employees to fire hazards in the event of ignition of the isopropanol.
