OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards from the loss of natural gas fuel containment for a gas fired combustion turbine: a) 2800 Christian St, Philadelphia, PA - Combustion Turbine/Heat Recovery Steam Generator - On or about June 15, 2016 employees were exposed to fire and explosion hazards from the loss of containment and the ignition of natural gas fuel to the Siemens Combustion Turbine. One feasible means of abatement, among others, is to comply with section 5 of the National Fire Protection Association Standard for the Installation and Use of Stationary Gas Engines and Combustion Turbines - NFPA 37 - 2015: Section 5 - Fuel Supply - Gaseous. Among other requirements, this section specifies the fuel gas supply shall contain two ASSV's (Automatic Safety Shutoff Valves), a manual leak test for each ASSV or an alternative means of providing the full closure of the ASSV, and both ASSV's shall stop the flow of fuel if the engine must be shut down due to abnormal or emergency operating conditions as specified by the manufacturer.
