(a) East side of house - On or about June 30, 2016 employee using an N16 Air Nail Gun set at 100 psi to fasten metal straps to side of house 16 feet above ground was not wearing any eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Top and East side of building - On or about June 30, 2016 employees building a 3 story wood framed house on an elevated foundation were walking on top of wood framed wall attaching metal tiedown straps on second floor 20 feet 9 inches high and attaching wood framed walls on the third floor 31 feet 2 inches high that were not protected with any type of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) North and west side of house - On or about June 30, 2016 employees framing a wood house gained access to upper levels using a single section of aluminum extension ladder that did not extend 3 feet above the landing and was not fastened at the top.
