29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    Pulaski Skyway Bridge, Newark to Kearny to Jersey City, NJ    The employer failed to protect employees from tripping hazards while walking around and over the following floor holes:    a)  Span 42 is 25 feet wide, with only 5 feet covered by lumber and an expansion joint approximately 2.5 feet wide and 18 inches deep, open 10 feet on one side of the plate and 13.5 feet on the other side.      Violation occurred from June 15-29, 2016
