29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about July 6, 2016, employees were exposed to being struck by falling objects. The employees were installing roofing materials on an existing commercial building located in Cleveland, Ohio 44128. The employee on the ground was working under the employees on the roof without wearing a protective helmet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about July 6, 2016, employees were exposed to a fall of approximately 16 feet. The employees were installing roofing materials on an existing commercial building located in Cleveland, Ohio 44128. The employees were using an extension ladder to access the roof. The ladder did not extend at least 3 feet over the landing area.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 6, 2016, employees were exposed to a fall of approximately 16 feet. The employees were installing roofing materials on an existing commercial building located in Cleveland, Ohio 44128. The employees were working on all areas of the roof without any form of fall protection. The Yoder's Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1075009, citation number 1, item number 1 and was affirmed as a final order on 7/20/2015, with respect to a workplace located in Brunswick, Ohio 44212.
