29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: Pulaski Skyway Bridge, Newark to Kearny to Jersey City, NJ The employer failed to protect employees from tripping hazards while walking around and over the following floor holes: a) Span 32 is 25 feet wide, with only 11 feet covered by a metal plate, and an expansion joint approximately 2 feet wide and 18 inches deep, open 12 feet on one side of the plate and 2 feet on the other side. b) Span 39 is 25 feet wide, with only 6 feet covered by lumber, and an expansion joint approximately 2 feet wide and 18 inches deep, open 8 feet on one side of the plate and 10 feet on the other side. c) Span 41 is 25 feet wide, with only 1.5 feet covered by lumber, and an expansion joint approximately 2 feet wide and 18 inches deep, open 10 feet on one side of the plate and 13.5 feet on the other side. d) Span 42 is 25 feet wide, with only 5 feet covered by lumber, and an expansion joint approximately 2.5 feet wide and 18 inches deep, open 10 feet on one side of the plate, and 13.5 feet on the other side. Violation occurred on or about June 15-29, 2016
