29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Moreno's Roofing and Concrete LLC project site located at 4123 Superior Ave. Sheboygan, WI: On July 5, 2016, employees were exposed to multiple hazards associated with roofing work, to include but not limited to, falls from elevated surfaces and ladders, lack of appropriate PPE, and damaged extension cords, when they had not been trained to recognize and avoid or correct those hazards or to understand and follow the OSHA Standards that apply to construction work.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:      Moreno's Roofing and Concrete LLC project site located at 4123 Superior Ave. Sheboygan, WI: On and before July 6, 2016, employees had not been trained to recognize and avoid or correct fall hazards, or to understand and follow the OSHA Standards that apply to fall protection, as evidenced by:  A) Employees working on the roof with varying pitches and eave heights were not using fall protection;  B) Employees were working around and near skylights that were not covered to prevent falling through them.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:     Moreno's Roofing and Concrete LLC project site located at 4123 Superior Ave. Sheboygan, WI: On and before July 6, 2016, employees had not been trained to properly set up and use ladders, in that:  A) A flexible ladder used to access the front porch roof did not extend at least 3' above the eave edge;  B) A flexible ladder used to access the roof on the west side of the house was leaned against the gable edge of the upper roof which did not provide equal support to both side rails of the ladder.
