29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: On or about June 8, 2016, and at times prior thereto, the hole cover installed on the duct work hole at the 2nd level was not secured to prevent it being displaced, exposing employees to a fall hazard of approximately 12 feet.
