29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    Koch Pool, Deerfield Academy, Deerfield, MA     On July 1, 2016, employees were exposed to an unprotected 14-foot fall into an empty swimming pool after one section of temporary guardrail was removed to facilitate the lowering of a piece of equipment into the pool.

The employer had not developed a written respiratory program.

The employer had not provided a medical evaluation for the employees who wore respirators.

The crane was loaded beyond its rated load:    Koch Pool, Deerfield Academy, Deerfield, MA     On July 1, 2016, employees were exposed to hazards when they were lowering a 220 pound piece of equipment using a scaffold-mounted swivel hoist crane that was rated by manufacturer to have a 100 pound maximum capacity.
