29 CFR 1910.95(c)(1): The employer had not administered a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent:  a.  On or about July 21, 2016, the pendulum saw operator was exposed to continuous noise levels at 79.4% of the action level (8-hour time-weighted average sound level of 85 dBA) or equivalent sound level of approximately 89.7 dBA during the 400 minute sampling period.  Exposure calculations include a zero increment for the 80 minutes not sampled.  b.  On or about July 21, 2016, the panel saw operator was exposed to continuous noise levels at 102.1% of the action level (8-hour time-weighted average sound level of 85 dBA) or equivalent sound level of approximately 91.5 dBA during the 394 minute sampling period.  Exposure calculations include a zero increment for the 86 minutes not sampled.  c.  On or about July 21, 2016, the crate assembler, east building, center assembly area, was exposed to continuous noise levels at 70.1% of the action level (8-hour time-weighted average sound level of 85 dBA) or equivalent sound level of approximately 88.9 dBA, during the 392 minute sampling period.   Exposure calculations include a zero increment for the 88 minutes not sampled.  d.  On or about July 21, 2016, the crate assembler, east building, north end, was exposed to continuous noise levels at 76.7% of the action level (8-hour time-weighted average sound level of 85 dBA) or equivalent sound level of approximately 89.6 dBA, during the 387 minute sampling period.  Exposure calculations include a zero increment for the 93 minutes not sampled.  e.  On or about July 21, 2016, the pallet/crate assembler, east side of saws building, was exposed to continuous noise levels at 94.6% of the action level (8-hour time-weighted average sound level of 85 dBA) or equivalent sound level of approximately 91.2 dBA, during the 382 minute sampling period.  Exposure calculations include a zero increment for the 98 minutes not sampled.
