29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:  a- Building Exterior: The scaffold work platform used by an employee consisted of only 1 plank as full planking was not utilized exposing the worker to fall hazards of approximately 12ft to the concrete below. 29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  a- Building Exterior: Employees were exposed to scaffold collapse as 2 of 8 cross braces were missing from the second section of tubular welded frame scaffold. Condition noted on or about 07/02/2016   Three Brothers Construction, LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.452(c)(2):  which was contained in OSHA Inspection Number 980850, Citation # 1, Item # 2, issued on 06/25/2014 with respect to a work place located at 590 Union Boulevard, Totowa NJ 07512.
