29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months: a) Jobsite, 220 Brady Street, New Brighton, PA 15066: Observed on or about June 22, 2016, a complete periodic inspection of each overhead crane being used at the facility had not been conducted in the past 12 months. Crane inspection documentation maintained by the employer indicated that the last periodic inspections of the overhead cranes were conducted on May 8, 2015. The periodic crane inspections were overdue by more than six weeks. Crane operators and employees working in areas of the plant where the cranes were being used were exposed to struck-by and crushed-by hazards.

This deficiency was identified during previous periodic inspections of the crane. Employees using the crane were exposed to struck-by and crushed-by hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: a) Bay 5, wall of building, 220 Brady Street, New Brighton, PA 15066: Observed on or about June 22, 2016, the nozzle on a compressed air gun located near a work station along the wall in Bay 5 was not capable of reducing the air pressure to less than 30 pounds per square inch (p.s.i.). The air pressure was set to approximately 108 pounds per square inch (p.s.i.) at the time of the inspection. Employees using the air gun were exposed to the hazard of embolism. b) Bay 5, near compressor room entrance, 220 Brady Street, New Brighton, PA 15066: Observed on or about June 22, 2016, the nozzle on a compressed air gun located near compressor room was not capable of reducing the air pressure to less than 30 pounds per square inch (p.s.i.). The air pressure was set to approximately 108 pounds per square inch (p.s.i.) at the time of the inspection. Employees using the air gun were exposed to the hazard of embolism.

The electrical equipment housed a main disconnect (circuit 8). Access to the electrical equipment was restricted by the storage of a fan, welding machine and other items in front of the electrical panel.
