29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about July 6, 2016, the employer failed to evaluate each authorized forklift operators performance at least once every three years. Employees were operating powered industrial vehicles to move supplies and outbound material. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
