29 CFR 1910.1200(f)(9): The employer did not ensure that labels or other forms of warning were legible, in English, and prominently displayed on the container, or readily available in the work area throughout each work:  a)  Kitchen;  On or about July 7, 2016 and at times prior thereto, the employer did not ensure that spray bottle containing Windex used by employees in the kitchen area had legible labeling to ensure the contents of the container was known.  A & Z, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(f)(9), which was contained in OSHA inspection number 1140939, citation number 1, item 1 and was affirmed as a final order on 05/31/2016.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  Kitchen;  On or about July 6, 2016 and at times prior thereto, the employer did not ensure that employees who use hazardous chemicals (such as but not limited to oven cleaners, bleach, and soaps) in the workplace for cleaning purposes had been provided with information and training on these chemicals.  A & Z, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 1140939, citation number 1, item 3 and was affirmed as a final order on 05/31/2016.
