OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were potentially exposed to chemical burns and inhalation hazards from an uncontrolled release of anhydrous ammonia:    a) Facility:  Ice formations on the piping, valves and equipment were not removed and the conditions causing the ice build-up were not corrected,  on or about 7/6/2016. See IIAR 2009, 4.10.7     b) Facility, throughout the ammonia system: The employer did not take measures to limit potential for corrosion of pipes and equipment which handled anhydrous ammonia and had no inspection and/or testing procedures to evaluate the mechanical integrity of pipes and equipment which handled anhydrous ammonia . Ice build-up was visible throughout the system, on or about 7/6/2016.  See IIAR B110.    c) Facility, on the ammonia system: The employer did not perform inspections, tests, or replace pressure relief valves on a scheduled basis to ensure the pressure relief valves were maintained in good working order See IIAR B110.    d) Facility, on the ammonia system: The employer did not complete a process hazards analysis (PHA) on the ammonia refrigeration system as described in IIAR B110, on or about 7/6/2016.    e) Facility: The employer only had one Scott full face respirator with dual cartridges. The employer failed to have self-contained breathing apparatus available for employee use with a secondary (backup) self-contained breathing apparatus available, on or about 7/6/2016.  See IIAR B109    f) Facility, Engine Room: The ventilation systems could only be operated manually and was not designed to be automatically activated in the event of a release by refrigerant detector(s) and temperature sensors , on or about 7/6/2016. See IIAR 2 13.3.1    g) Facility, Engine Room:  The employer did not install dual relief assemblies on the vessels, on or about 7/6/2016. See ASHRAE 15 Safety Standards for Refrigeration Systems paragraph 9.7.2.3    h) Facility: Piping was not labelled with contents, directional flow, on or about 7/6/2016. See IIAR Bulletin 114    i) Facility: The employer failed to develop a piping and instrumentation diagram/drawing (P&ID), to help identify the location of ammonia refrigeration system equipment in the facility that was encased in ice, on or about 7/6/2016. See ANSI/ISA S5.1.
