29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about July 6, 2016, at a worksite located at 116 Tallmadge Circle, Tallmadge, OH 44278, during construction activities including but not limited to setting light poles with a truck mounted hydraulic crane, employees were exposed to possible head injuries from impact or falling objects.

29 CFR 1926.1404(q)(2): 29 CFR 1926.1404(q)(2): The outriggers were not set to remove the equipment weight from the wheels:    On or about July 6, 2016, at a worksite located at 116 Tallmadge Circle, Tallmadge, OH 44278, during construction activities including but not limited to setting light poles with a truck mounted hydraulic crane, employees were exposed to struck by and impact hazards when the outriggers were not properly set to remove the weight from the wheels.
