On or about July, 7, 2016 and at times prior thereto, at the construction site located at or near 6926 Woodhaven Circle, Fort Smith, Arkansas, an employee used a pneumatic nail gun without eye protection, exposed to eye hazards while performing roofing construction.

On or about July 5, 2016 and at times prior thereto, at the construction site located at or near 6926 Woodhaven Cir, Fort Smith, Arkansas, employees were performing residential roofing construction at heights greater than 6 feet without fall protection. Employees were exposed to the hazard of falling to the ground below.
