29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:  a) At the workplace located at 111 North 14th Street, Coshocton, Ohio, specific energy control procedures for the Fab-Tube machine had not been developed and implemented to protect machine operators when clearing jams and performing machine adjustments. On or about July 6, 2016, a Fab-Tube machine operator suffered the amputation of three fingers while attempting to clear a jam and make machine adjustments in the mandrill area of the Fab-Tube machine.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about July 6, 2016 , the Fab-Tube operator suffered the amputation of three fingers while attempting to clear a jam in the mandrill area of the Fab-tube machine. The operator had not received training on the means to ensure that all power sources, to include but not limited to the air supply, had been isolated.

An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.  a) On or about July 6, 2016, an employee suffered the amputation of three fingers while attempting to perform machine adjustments and clear a jam in the mandrill area of the Fab-Tube machine. The employer failed to implement energy isolating procedures to ensure that all energy sources had been shut down or turned off prior to performing servicing work.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  a) On or about July 6, 2016, an employee suffered the amputation of three fingers while attempting to clear a jam and perform machine adjustments in the mandrill area of the Fab-Tube machine. The employer failed to implement procedures to ensure that energy isolating devices were located in such a way as to ensure that all energy was isolated prior to breaching the Fab-Tube's barrier guard.
