Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   a) Roof - Employees removing existing asphalt lament on a flat roof that measured 20 feet by 34 feet were not protected from a fall of 17 feet, 3 inches, on or about 07/01-02/2016  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
