29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a) Rt 9 (Russell Rd) Hadley, MA: On or about July 6, 2016, two employees were working in an excavation greater than 5 feet deep without any protective system.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).    Caracas Construction Corp was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.652 (a) (1), which was contained in OSHA inspection number 991752, citation number 1, item number  1 and was affirmed as a final order on September 25, 2014, with respect to a workplace located on Rt 5 in Longmeadow, MA
