29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  (a)  Jobsite:  On or about July 2, 2016, the belt and pulleys on the Jenny air compressor were not guarded against employee contact.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Jobsite:  On or about July 2, 2016, two employees were working from a roof and they were not protected from falling off of the roof.  A third employee was improperly tied off because he was tied off at his one hip.  The pitch of the roof was 7 in 12 and the ground to eave height was about 28 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:   (a)  Jobsite:  On or about July 2, 2016, the three employees that were working from the 7 in 12 pitched roof need to be retrained in how to minimize fall hazards when working from a roof.  The fall  hazard to the three employees was about 28 feet to the ground.
