29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    On or about July 7, 2016 workers were engaged in commercial construction activities installing reinforcing rods in a commercial concrete structure. Workers were exposed to potential struck-by and fall protection hazards in that the workers were not properly trained in construction safety criteria such as fall protection training, rigging and signaling requirements, fork lift training, in that the company provided OSHA falsified documentation showing employees training.
