29 CFR 1926.651(k)(1):  An inspection of the excavation, the adjacent areas, and protective systems was not conducted by a competent person prior to the start of work and as needed throughout the shift:      The employer did ensure the inspections were conducted by a competent person who has the knowledge to identify hazards associated with working in an excavation.  An employee was exposed to cave-in hazards when working in an unprotected excavation approximately 10 feet in depth, approximately 5 feet wide, approximately 20 feet in length, and comprised of material consistent with that of a Type B soil.
