29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:    On or about June 30, 2016, the employer failed to ensure that the storage and utility area above the bathroom area was equipped with standard railings and toeboards. The area did not contain a mid rail and sections contained no railings or toeboards.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    On or about June 30, 2016, the employer failed to ensure the workplace and tasks performed by employees were assessed to determine the hazards present. Employees engaged in the handling and loading of coiled steel were not provided cut resistant hand protection for such task.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  On or about June 17, 2016, the employer failed to ensure that appropriate cut resistant gloves were provided to employees tasked with the handling of raw steel coils. Employees were provided cotton gloves not capable of protecting the employee from the sharp edges of the steel coils.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    a) On or about June 30, 2016, the employer failed to ensure that employees were protected from the point of operation of the Niagara E-110 press (#23). Guarding was not present on the front of the press while running in fully automatic mode. The company merely provided plastic chain links on sides of press for operator awareness. Operators still had the ability to reach to hazardous area.    b) On or about June 30, 2016, the employer failed to ensure that employees were protected from the point of operation of the Bliss C-150. Barrier guarding was in place however openings were large enough for employees to reach to hazardous area.
