OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling from a fiberglass extension ladder while attempting to access/egress steel beams or elevated scaffold planks. (a) 271 Suedberg Road, Pine Grove, PA - An unsecured twenty-foot (20') fiberglass extension ladder was placed against a concrete wall and was used by employees for access/egress to a steel beam (eighteen-foot, eight-inches - 18'8" - above ground level) and for access/egress to three (3) elevated aluminum scaffold planks (twelve-foot, eight-inches - 12'8" - above ground level) which created side loads that could destabilize and/or shift the extension ladder exposing the employees to a fall hazard, as observed on or about July 8, 2016.
