29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) 1115 Highland Colony Parkway in Ridgeland, MS - On or about July 7, 2016 an employee climbed out of an aerial lift basket onto the roof peak and did not have any fall protection.

29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:  (a) 1115 Highland Colony Parkway in Ridgeland, MS - On or about July 7, 2016 an employee was wearing a body harness and the lanyard was wrapped around the front top guard rail and was not attached to an approved anchor point.
