29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about June 30, 2016, an employee was welding in the steel stair case for the condominiums under construction using an aerial lift up to approximately 30 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about June 30, 2016, welders were installing the steel stair case on the residential condominiums under construction up to approximately 65 feet high without fall protection, exposing the employees to a fall hazard.
