29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about July 1, 2016, employees were framing the hotel under construction approximately 48 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer did not provide a training program for all employees exposed to fall hazards.  The program shall include training and instruction in the following areas: a)  The recognition and identification of fall hazards in the work area; b)  The use and operation of guardrail systems (including perimeter safety cable systems), personal fall arrest systems, positioning device systems, fall restraint systems, safety net systems, and other protection to be used; c)  The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; d)  The procedures to be followed to prevent falls to lower levels and through or into holes and openings in walking/working surfaces and walls; and e)  The fall protection requirements of this subpart.
