29 CFR 1910.27(b)(7)(i): Metal ladders and appurtenances were not painted or otherwise treated to resist corrosion and rusting when location demands: (a) Roof of 120 Centerville Road, Warwick, RI: On or about July 8, 2016, the two rungs on the individual-rung ladder were not adequately treated to resist rusting when located out in the elements, in that the paint was peeled off and the metal was rusting in areas such as, but not limited to, the attachment bolts, the attachment plate and the individual rungs themselves.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition, wherever it is necessary by reason of hazards of processes or environment, encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: (a) Roof of 120 Centerville Road Warwick, RI: On or about July 8, 2016, an employee performing work up on the roof, servicing air conditioning equipment, was not provided with adequate fall protection for use when traversing from multi-level flat and steep roofs and while working on flat roofs that do not have guardrails.
