The employer shall determine, through regular supervision and through inspections conducted on at least an annual basis, that each employee is complying with the safety related work practices required by this subpart. The employer had not provided adequate, regular supervision to prevent an electrocution hazard to crew members on the five and one half week Blackhawk Conversion 4-34.5 kv Phase 2 Project in that no Foreman was available to know whether employees were complying safety-related work practices.

The briefing shall cover at least the following subjects: Hazards associated with the job, work procedures involved, special precautions, energy-source controls, and personal protective equipment requirements. On July 5, 2016, the pre-job briefing conducted by the Crew Foreman was inadequate in that it did not address the hazards of removing a wood pole and what specific length of cover or personal protective equipment to be worn by a crew member was required near the energized 19.9 kv transmission line at 600 Watkins-Bowman Road, Blanchester, OH to prevent an electrocution hazard.

29 CFR 1926.960(c)(1)(iii): The employer shall ensure that no employee approaches or takes any conductive object closer to exposed energized parts than the employer's established minimum approach distance, unless: On July 5, 2016 at around 600 Watkins-Bowman Road, Blanchester, a crew member was exposed to an electrocution hazard from an energized 19.9kv powerline when the employee came in contact with the copper conductor on the wood utility pole while the pole was being removed and came within the minimum approach distance of 2 ft. 1 inch.

29 CFR 1926.964(a)(3)(ii): When a pole is set, moved, or removed near an exposed energized overhead conductor, the employer shall ensure that each employee wears electrical protective equipment or uses insulated devices when handling the pole and that no employee contacts the pole with uninsulated parts of his or her body. a. On July 5, 2016, at 600 Watkins-Bowman Road, Blanchester, a crew member was exposed to an electrocution hazard while the crew removed the first wood utility pole which could come in contact with a energized 19.9kv transmission line without the sufficient length of cover on the energized transmission line to prevent the pole from contact with the energized transmission line. b. On July 5, 2016 at 600 Watkins-Bowman Road, Blanchester, a crew member was exposed to an electrocution hazard while the crew removed the second wood utility pole which came in contact with a energized 19.9kv transmission line and without the sufficient length of cover on the energized transmission line to prevent the pole from contact with the energized transmission line.
