29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite, 1342 William Penn Avenue, Johnstown, PA:  On or about July 11, 2016, employees were exposed to a fall hazard while using a MEC 5492RT scissors lift to come down from the roof they were working on, because the gate was not closed, therefore it did not provide a fully enclosed guardrail system and no other means of fall protection were utilized.  The height of the roof was approximately 45 feet above the ground.

29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:  a) Jobsite, 1342 William Penn Avenue, Johnstown, PA:  On or about July 11, 2016, three employees were exposed to fall hazards because warning lines were not erected on the roof edge on 2 sides of the 69 foot by 134 foot section of flat roof they were working on.  The fall hazard to the employees was approximately 45 feet to the ground.
