29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected for each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  a) Work site was located at 2419 Kennedy Blvd, Jersey City, NJ 07304  Employees were exposed to electrical hazards as they were working with a DeWalt D25213 Hammer Drill, Serial Number 080355, with a cord on it that was separated from the outer insulation at the strain relief exposing the wiring itself.  Violation was observed on or about 6/24/2016.

29 CFR 1926.501(c)(3): Barricade the area to which objects could fall, prohibit employees from entering the barricaded area, and keep objects that may fall far enough away from the edge of a higher level so that those objects would not go over the edge if they were accidentally displaced.  a) Worksite was located at 2419 Kennedy Blvd. Jersey City, NJ 07304  Employees were exposed to struck by hazards as they had stored tools, chemicals and other materials on the parapet wall extending over the leading edge where it could easily fall to the ground below with incidental contact potentially striking employees entering the stairway to the roof from the front sidewalk or working in the courtyard where the trucks and lift were staged.  Violation was observed on or about 6/24/2016.

29 CFR 1910.1200(b)(4)(ii): In work operations where employees only handled chemicals in sealed containers which were not opened under normal conditions of use, the employer did not ensure that the material safety data sheets were readily accessible during each work shift to employees when they are in their work area.    a) Work site was located at 2419 Kennedy Blvd. Jersey City, NJ 07304    Employees were exposed to chemical hazards while using chemicals in the work place without having the MSDS present on site for familiarization of chemical hazards associated with those products. Products in use on site included Acetone, Sikaflex, EverGuard Water-Block, and Greased Lightning.    Violation was observed on or about 6/24/2016.    b) Work site was located at 2419 Kennedy Blvd., Jersey City, NJ 07304    Employees were exposed to chemical hazards as they were working with chemicals such as Acetone without wearing eye protection.    Violation was observed on or about 6/24/2016.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    a) Work site was located at 2419 Kennedy Blvd. Jersey City, NJ 07304    An employee was injured in a fall on the jobsite that required him to be hospitalized at Jersey City Medical Center for a period of three days  starting on 6/15/2016 that was not reported to OSHA by the employer.    Violation was observed on or about 6/22/2016.
