29 CFR 1926.1101(k)(2)(ii): Building and/or facility owners did not notify the contractors of the presence, location and quantity of ACM or PACM, at the work sites in their buildings and facilities:      a)  4632 Centre Avenue, Pittsburgh PA, 1st Floor Apartment Complex - On or about July 8, 2016, Employees performed Class II asbestos construction activity including hammering and sawing asbestos containing floor tiles, asbestos containing wall board and asbestos containing ceiling surface during demolition, clean out and removal.      The building owner did not conduct an asbestos survey to notify the demolition contractor of the presence of asbestos containing material in the floor and ceiling tile of the multi level building.
