29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years: On 07/12/2016, employees working at Delden Manufacturing Co., Inc. dba Renner Supply Co. located at 402 St. Louis Ave., Valley Park, MO 63088, were operating forklifts without having completed the required refresher training. Company records had shown the operators had attended forklift training on May 9, 2007, but had not completed the 3 year refresher training/evaluation since.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: Delden Manufacturing Co., Inc. dba Renner Supply Co., located at 402 St. Louis Ave., Valley Park, MO 63088, did not maintain their 2016 Year-To-Date OSHA 300 log up-to-date. There were two recordable incidents which were not on the log. These occurred on 06/03/2016 and 03/21/2016.
