29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Multiuse new construction at 90 Ocean Terrace in Long Branch, NJ, 07740- Route 36 side of building: Employees worked on a Genie boom lift, model Z60/34, serial number Z6011-10371 without the use of fall protection on or about 6/22/16. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
