29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:        a) House interior: The scaffold casters and wheels were not locked on a Perry Mobile scaffold to prevent movement while the scaffold was used. On 7/11/2016, an employee fell while working form a Perry Mobile scaffold that tipped over.
