29 CFR 1910.119(l)(2)(ii): 29 CFR 1910.119(l)(2)(ii): The management of change procedures did not assure that the impact of changes on safety and health were addressed prior to any change: a) Refinery at 4 Paradise Road, Paulsboro, NJ, 08066- Temporary NaSH operation from unit 3: Employees attempting to load and unload NaSH product via a temporary system attached to unit 3 were exposed to a hazardous atmosphere containing H2S. The employer failed to implement their written Management of Change (MOC) procedures when the impact on employee health and safety was not addressed for this temporary operation involving process equipment, on or about 5/27/16. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
