29 CFR 1910.37(a)(3): Exit routes were not free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor:  a.  On or about July 12, 2016, in the warehouse office, the exit route was not unobstructed, the exit door was blocked by supplies.

On or about July 12, 2016, in the warehouse along the exit route, doors that could be mistaken for an exit were not mark.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  a.  On or about July 12, 2016, at the warehouse, employees operating forklifts had not been practically trained and evaluated by the employer, exposing employees to a crush-hazard.
