29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Department 9 , 121 Crescent Street, Athol, MA: On or about July 12, 2016 employees were exposed to amputation from hazards from a Miller Model A63 SN#4116 Pneumatic bender which had point of operation hazards that were not guarded to prevent employees from having any part of their body in the danger zone during operating cycles. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
