29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: JOB SITE: 850 West Park Road, Union, MO 63084, on or about 7/11/2016, on F-27 Carousel, #T07064-4, the machine guard was not provided to protect the operator from an ingoing nip point hazard as the worker attempt to remove a paper jam. The worker jogged the rollers without the guard to align the paper in-place, the worker's left middle & ring fingers were caught between the rollers. The worker sustained an amputation injury. R.R. Donnelley & Sons Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.212(a)(1), which was contained in OSHA inspection number 1130304 citation number 1, item number 3, and was affirmed as a final order on 4/15/2016, with respect to a workplace located at 1005 Commercial Drive, Owensville, MO 65066.
