A) Interior, Second Floor: Employees were exposed to fall hazards of approximately 12 feet while engaged in demolition work on the upper floor of a structure. The floor in which they were walking/working on had holes in it in which they could fall to the lower level. None of the employees had any type of fall protection in place, on or about June 16th, 2016.   Trident Structural Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(4)(i), which was contained in OSHA inspection number 911160, citation number 1, item number 3 and was affirmed as a final order on November 6th, 2013, with respect to a workplace located at 231 W. 22nd St in Manhattan, New York.
