29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   The employer is failing to protect employees from serious struck by hazards. This was most recently documented at 9327 North East 79th Street Kansas City, Missouri 64158 Building 11. Employees were observed using an electric saw without the use of eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  The employer is failing to protect employees from fall hazards. This was most recently documented at 9327 North East 79th Street Kansas City, Missouri 64158 Building 11. Employees were exposed to a fall hazard of approximately 30 feet while working over the edge of the 3rd story apartment building with a roof pitch of 6.
