29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about July 13, 2016, the employer failed to establish a Control of Hazardous Energy program and train employees on the safe performance of servicing and maintenance of various machinery to include but not limited to: Smart Products Dismantler/Stripper Saw, MSI Trim Saw and the Automated Machine Systems Board Saw. Employees are exposed to hazardous such as moving parts and point of operation (saw blade) in the event a machine is accidentally started back up while being serviced.

Employees are exposed to various parts of the saw blade which spans the width of the dismantler table when the machine is in operation, including at the point of operation.
