29 CFR 1910.95(b)(1):  When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:    a.  On or about July 28, 2016, the Dublin trim saw operator was exposed to continuous noise levels at 215.9% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA) during the 461 minute sampling period.  Exposure calculations include a zero increment for the 19 minutes not sampled.    b.  On or about July 28, 2016 the pallet nail gun operator was exposed to continuous noise levels at 690.9% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA) during the 469 minute sampling period. Exposure calculations include a zero increment for the 11 minutes not sampled.    c.  On or about July 28, 2016, the Teardown/Dismantler operator was exposed to continuous noise levels at 546.0% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA)  during the 469 minute sampling period. Exposure calculations include a zero increment for the 28 minutes not sampled.      d.  On or about July 28, 2016, the pallet nailer operator was exposed to continuous noise levels at 336.14% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA)  during the 452 minute sampling period. Exposure calculations include a zero increment for the 28 minutes not sampled.

29 CFR 1910.95(c)(1): The employer had not administered a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent:    a.  On or about July 28, 2016, the Dublin trim sawoperator was exposed to continuous noise levels at 222.9% of the action level (8-hour time-weighted average sound level of 85 dBA) during the 461 minute sampling period.  Exposure calculations include a zero increment for the 19 minutes not sampled.    b.  On or about July 28, 2016, the pallet nail gun operator was exposed to continuous noise levels at 696.7% of the action level (8-hour time-weighted average sound level of 85 dBA) during the 469 minute sampling period.  Exposure calculations include a zero increment for the 11 minutes not sampled.    c.  On or about July 28, 2016, the tear down/dismantler operator was exposed to continuous noise levels at 553.6% of the action level (8-hour time-weighted average sound level of 85 dBA) during the 469 minute sampling period.   Exposure calculations include a zero increment for the 28 minutes not sampled.    d.  On or about July 28, 2016, the pallet nail gun operator was exposed to continuous noise levels at 344.3% of the action level (8-hour time-weighted average sound level of 85 dBA) during the 452 minute sampling period.  Exposure calculations include a zero increment for the 28 minutes not sampled.    e.  On or about July 28, 2016, the chop saw/trim saw operator was exposed to continuous noise levels at 66.4% of the action level (8-hour time-weighted average sound level of 85 dBA)  during the 113 minute sampling period.
