29 CFR 1910.136(a): General requirements. The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures. a) The employer did not provide and require the employees to wear foot protection, in that during the movement of paper rolls from the pallets exposes employees to foot injury.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) The racking system in the warehouse were not stable and secured to prevent material from sliding, falling and/or collapsing in that the material stored on the racks were not stacked, blocked, interlocked and limited in height to prevent employees to struck by hazards.
